Requirements for PCS Fellowship

• Accomplished Application Form.
• Photocopy of updated/valid Professional Identification Card issued by PRC.
• Photocopy of the Certificates of Completion of Residency and Post-Residency Specialty Training.
• Certified true copy of Specialty Board Certification or Letter of certification from the Specialty Board (for foreigners).
• Photo copy of the latest Philippine Medical Association (PMA) ID.
• List of ALL previous and current hospital (s) of practice. Includes: Name, Contact Number and Email Address of the Medical Director or Chief of Hospital, Chair or Department of Surgery or Chief, Specialty Division
• Letters of recommendation from two (2) PCS Fellows in good standing attesting to his/her good moral character and high ethical and professional standing in the community (predominant area/hospital of practice).
• Letter of endorsement from the PCS Chapter in his/her area of practice.
• Letter of endorsement from his/her respective PCS Component Specialty Society.
• Three (3) 2 x 2 pictures (colored with white background; coat and tie or barong).
• Application fee of Php2,500 for Filipino citizens and USD500 for foreign nationals. Application fee for deferred applications shall remain valid only until the following year.
• Interview of the applicant as deemed necessary by the Committee.

PCS Simplified Application for Surgical Specialty Society Members

Qualification: Fellows of PCS component Specialty Society for at least 3 years

• Accomplished Application Form.
• Photocopy of updated/valid Professional Identification Card issued by PRC.
• Letter of endorsement from the PCS Chapter in his/her area of practice.
• Letter of endorsement from his/her respective PCS Sub-Specialty Society.
• Three (3) 2 x 2 pictures (colored with white background; coat and tie or barong).
• Application fee of Php 2,500. (Application fee for deferred applicants shall remain valid only until the following year)
• Interview of the applicant as deemed necessary by the Committee.
